The U.S. Consumer Product Safety Commission, in cooperation with Evenflo Inc., has announced the voluntary recall of certain convertible high chairs. These chairs were sold in Toys R Us and Walmart stores nationwide and online at walmart.com and wayfair.com between December 2011 and June 2012.
The activity tray on the high chair can unexpectedly detach, allowing an unrestrained child to fall and possibly be injured.
Evenflo has received 18 reports of trays that have detached and eight reports of injuries resulting from children falling from the high chair.
The recall involves high chairs that convert from a high chair to a toddler-size table and chair. Model numbers are located on a label on the lower portion of on the high chair's legs.

Consumers should immediately contact Evenflo at (800) 233-5921 between 8 a.m. and 5 p.m. ET Monday through Friday, for a replacement tray with installation and use instructions. Consumers can also visit Evenflo's website at http://safety.evenflo.com.
