MOBILE, AL (NBC) -

Faulty equipment is blamed for a deadly accident involving a cruise ship which broke free from a dock on April 3, 2013.

Video showed the Carnival Triumph slamming into another ship during a windstorm last april while it was docked at the BAE Systems Shipyard in Mobile.

A dock worker was thrown into the water and drowned. A National Transportation Safety Board blames faulty "bollards", which were used to secure the ship to the dock.  

But shipyard managers disagree. BAE Systems executives want the NTSB to reconsider the cause saying there is new information about the sequence of events not available during the investigation.

A spokesperson says the company may submit "metallurgical and meteorological assessments" which were not available during the fact-finding phase of the board's investigation.
